Located in Chuo Ward, Sapporo, about a 7-minute walk from Nakajima Koen Station on the Subway Namboku Line, Kaede no Mori Clinic is a clinic providing comprehensive medical services centered on internal medicine. In addition to outpatient care focused on general internal medicine and fever clinics, the clinic offers home medical care, vaccinations (reservation required), and self-pay COVID-19 PCR testing. The Department of Rehabilitation provides dysphagia treatment and botulinum therapy, as well as home and facility-based rehabilitation by physical therapists, supporting diverse rehabilitation needs of local residents. As a certified enhanced home care support clinic, it also serves as a base supporting home medical care in the community. The clinic also accepts occupational physician services for nearby companies, contributing to the health management of workers. English-speaking staff are available, and various payment methods including credit cards, debit cards, and electronic payments are accepted. Parking and barrier-free facilities are available. The clinic is closed on Tuesdays, Sundays, the third Thursday of each month, holidays, and year-end/New Year holidays. On Thursdays, outpatient care is not provided, only home visit services. Same-day appointments are available if there are openings, but the clinic may be closed during home visits, so prior confirmation is recommended.
