Located in Kita 45-jo Higashi, Higashi-ku, Sapporo, Teishinkai Kita 45-jo At-home Clinic is an internal medicine clinic operated by the Social Medical Corporation Teishinkai, which was founded in 1984. Based on the philosophy of "contributing to the prosperity of the local community through high-quality medical and nursing care," the corporation currently operates 26 facilities that integrate medical and nursing services in the Sapporo and Soya regions. This clinic was established as a facility providing community-based home medical care. The core service of this clinic is home-visit medical care, where physicians visit patients who have difficulty visiting the clinic to provide internal medicine services. In cooperation with the corporation's nursing care facilities, the clinic also offers visiting rehabilitation, visiting nursing, and home care services, providing continuous support for patients' home lives from both medical and nursing care perspectives. The clinic can provide services in English, and with barrier-free design and on-site parking, it offers an accessible environment for people in various situations. Medical services are available on weekdays, and the clinic is closed on Saturdays, Sundays, public holidays, and year-end/New Year holidays (December 30 to January 3).
