Kanta Internal Medicine Clinic, located in Onojo City, Fukuoka Prefecture, is a clinic that provides integrated services including outpatient internal medicine, home visit care, home medical care, and palliative care home hospice. The clinic director graduated from Fukuoka University School of Medicine, gained experience in gastroenterology, emergency medicine, and palliative care wards, and established the clinic in June 2016. A key feature of the clinic is its two-track approach: morning outpatient consultations for patients who are able to visit, and afternoon home visits for those who have difficulty traveling to the clinic, serving as a trusted family doctor in the community. For home visit services, the coverage area is limited to within a 3km radius to minimize travel time and maintain quality of care, and the number of patients under care is kept to a manageable level to ensure safety and quality. The clinic also places emphasis on palliative care to support the best possible end-of-life care for all patients, including those with cancer, practicing home hospice that respects the wishes of patients and their families. Patients aged 15 and above are accepted. Outpatient hours are Monday, Tuesday, Wednesday, and Friday mornings from 9:00 to 12:30. Consultations in English are available, credit cards and electronic payments are accepted, and the facility is equipped with parking and barrier-free access.
