In addition to comprehensive support through multidisciplinary collaboration including home nursing and rehabilitation services, they provide consultation services related to disability welfare services, offering continuous care that takes the living environment into consideration. Through consultation support offices, they facilitate connections to disability welfare services and coordinate with relevant organizations.
