Minamino Cardiovascular Hospital is a specialized internal medicine hospital located in Hyoe, Hachioji City, Tokyo. The hospital also focuses on research activities, conducting studies on unsupervised exercise therapy for elderly heart failure patients who cannot participate in outpatient cardiac rehabilitation. This research aims to clarify the effectiveness of home-based exercise therapy for elderly heart failure patients whose physical function has declined due to hospitalization or aging. The exercise program is developed in accordance with guidelines from the Japanese Circulation Society and the Japanese Association of Cardiac Rehabilitation, providing instruction on aerobic exercise, balance training, and strength training. The study targets heart failure patients aged 65 and older who are attending outpatient visits after discharge. With consideration for patient privacy protection, personally identifiable information is separated and used after anonymization. Closed days are limited to three days from January 1st to 3rd.
