Aozora Clinic Kochi Ushioe, located in Kitatakejima-cho, Kochi City, Kochi Prefecture, is a home care support clinic that provides home visits and house calls with 24-hour response. Established in February 2009 in Kochi City, the clinic has worked to promote home medical care, and will begin new activities as "Medical Corporation Sora no Iro Aozora Clinic Kochi Ushioe" from April 2025. Patients eligible for care include those with age-related diseases such as dementia and cerebrovascular disorders, those with internal organ diseases such as heart failure, kidney failure, and respiratory diseases, those with progressive neurodegenerative diseases, those with severe disabilities from childhood requiring medical care, those requiring home palliative care including terminal cancer, and those wishing to pass away at home. The clinic provides medical management possible in home care settings, including enteral nutrition, parenteral nutrition, oxygen therapy, tracheostomy, artificial ventilation management, and stoma and catheter management. To promote and raise awareness of home medical care, the clinic engages in public activities, accepting students from Kochi University Medical School and Kochi Prefectural University School of Nursing, clinical residents, and visits and training from multidisciplinary professionals, and is a training facility for specialists of the Japan Home Care Medical Association. In cooperation with local visiting nurse stations, the clinic supports people of all ages from children to the elderly to live safely at home, and operates as a barrier-free facility with an English-friendly parking area. With the philosophy of supporting "your own life" so that people with chronic illnesses or disabilities, those requiring medical care, and those with limited time can live their precious time, in precious places, with precious people, the clinic is dedicated to its mission.
