Located in Kita-Hokumachi, Muroran City, Hokkaido, Honwanishi Family Clinic is a community-based medical clinic specializing in internal medicine. Established by the Hokkaido Center for Family Medicine (HCFM), the clinic provides group medical care by multiple physicians, including family medicine specialists and certified instructors. Since February 2026, the clinic has transitioned from an appointment-based system to a first-come, first-served system; patients can be seen by visiting directly during reception hours (8:30 AM–11:00 AM and 2:15 PM–4:30 PM; closed on Wednesday and Saturday afternoons). In addition to outpatient care, the clinic offers home visit services. While regular home visits are provided twice a month for patients with difficulty visiting the clinic, a 24/7 system is in place to respond to acute and emergency situations. The clinic actively collaborates with local clinics and nearby general hospitals through clinic-hospital and clinic-clinic partnerships, providing continuous care that includes referrals back from transferred facilities. Additionally, since 2021, the clinic has been continuously advancing the Advance Care Planning (ACP) project, which enables patients to comfortably discuss healthcare and care in the final stages of life.
