Ishii Home Care Clinic, located in Koriyama City, Fukushima Prefecture, is a medical institution specializing in home visits and house calls, established in May 1998 as a dedicated home care clinic. Dr. Tomonori Ishii, the clinic director, graduated from Tohoku University School of Medicine and is a member of the Japanese Society of Neurology, specializing in internal medicine and neurology. The clinic serves patients who have difficulty visiting hospitals independently, including those with post-stroke sequelae, Alzheimer's disease, neurodegenerative diseases such as ALS, and terminal cancer. Currently, the clinic provides home visits to approximately 600 patients at home or in facilities, with about 250 end-of-life care cases annually. It supports a wide range of medical procedures including home oxygen therapy, tube feeding via gastrostomy, home mechanical ventilation, central venous nutrition, and opioid management (including PCA pumps), offering comprehensive support for home care of patients with serious conditions. The clinic also utilizes "Zaitaku Care Net," a system for securely sharing medical information online with affiliated home nursing stations and care managers, practicing team-based care. English service is available, and the facility is equipped with parking and barrier-free access.
